Australian Visa Applicants
The quickest and easiest way to book your appointment is by completing the
Booking Request Form. (Click here to download)
Once you have completed this please:

Fax to: 01628 760915
Please then call us on 01628 760919 to make a convenient appointment.
Alternatively we will aim to call you back within 24 hours of receiving your details to discuss convenient appointment times.
*If you have not yet lodged your visa application please advise us of this on booking. This is very important as we have to process your application in a different way.
If you do not wish to submit a booking request in advance of telephoning that is acceptable. However on calling please be prepared to talk to us for 15 to 20 minutes and have to hand the following information:
- The names, dates of birth, addresses and contact of numbers of all attendees.
- Visa subclass applied for
- TRN number or HAPI D
- Your credit card details.
In advance of your appointment:
To save time on the day of your appointment it is useful if you can chose the Bridge Clinic and complete your health questionnaire online.
To do so please complete the following steps:
- Please visit: http://www.immi.gov.au/e_visa/check-progress.htm
- Please select the visa type you are applying for. This should then take you to a page that looks like this:

- Enter your details and click next.
- The next page shows you your application status.
- Please select Complete Health Forms for this Applicant at the bottom of the page.
- This enables you to access your Online Health record. Click next at the bottom of the page.
- Select the Bridge Clinic. Click next.
- Complete the medical history questions as prompted by the system and press submit.
- Please print your Health Examination List or Examination Referral Letter.
To your appointment you will need to bring
- Your printed Health Examinations List or Examination Referral Letter or Forms 26EH/160EH
- Passport
- A completed Immigration Medical Registration Form. Please complete one form for the individual or family (download here).
- Any helpful documentation you have regarding existing or previous medical conditions
- Any prescription glasses or contact lenses you use.
Please note a urine sample will be required at some stage during the medical from all applicants of 5 years and older. Ladies should avoid booking appointments during their menstrual cycle as the urine sample may be contaminated. If this happens you will need to provide another specimen at a later date.
Following your appointment
Please be assured that we will always do our utmost to ensure that documents are completed and despatched as promptly as possible. However we cannot guarantee specific timeframes for processing medical documents as sometimes unforeseen circumstances can occur. This is especially pertinent for clients who require further third party medical opinion or additional tests.
It is not necessary for you to telephone the clinic for x-ray, urine or blood test results. We will contact you directly if there are any issues arising from your results. Your co-operation in this matter is very much appreciated
